Section 3026 funding)

The Centers for Medicare & Medicaid Services (CMS) is dedicating millions of dollars nationwide to communities focused on helping patients safely transition between care settings. Funding was made available on April 12, 2011 through the Community-based Care Transitions Program (CCTP).
